| Student | First name, last name, preferred name, student ID or NSN (optional), year level, form class, student email | To find the right student and send them their plan. The ID matches updates. |
| Student, optional | Subjects or classes, credits this year | Context for pathway conversations, such as NCEA credits. |
| Parents or caregivers (up to two) | Name, email, relationship | Only so you can choose to include them. |
| Staff | Form teachers' names and emails, plus anyone you refer students to (dean, Gateway coordinator, counsellor) with their role | So next steps and referrals reach the right person. |
